Navigating available benefits can be overwhelming. Connect to Care’s staff is certified to provide benefits counseling to help explain the different State and Federal benefits available.

In addition, our staff may assist in obtaining cost savings through Medicare and/or maximizing or accessing the benefits you may already have.

Individuals may receive assistance in applying for:

  • SNAP food benefits (food stamps)
  • Healthcare benefits (Medicaid, CHIP & Marketplace)
  • Cash help for families (Temporary Assistance for Needy Families - TANF)
  • Medicare Savings Program (MSP), Low Income Subsidy (LIS), ExtraHelp
  • Long-term Care Services and Supports
  • Federal Benefits for People with Disabilities (Social Security Disability Insurance -
    SSDI/Supplemental Security Income - SSI)
 

Community Partner Program

As a certified Community Based Organization (CBO) with the Health and Human Services Commission (HHSC), Connect to Care staff are certified navigators who can assist individuals in applying for and managing their HHSC benefits online, including:

  • Submitting a new application
  • Obtaining a renewal for existing services
  • Managing your case online
 

Certified Application Assister Site

As a Certified Application Assister Site for the Marketplace, our certified application counselors can help individuals:

  • Navigate healthcare coverage under the Marketplace
  • Look for health coverage options through the Marketplace
  • Complete eligibility and enrollment forms
 

Social Security Disability Insurance/Social Security Income

Our certified consumer benefits specialists can assist individuals with disabilities in applying for federal benefits, available through the Social Security Administration. Benefits specialists can help individuals:

  • Understand eligibility requirements
  • Apply for benefits
  • Appeal denials
